Add to https://github.com/davidtaylorhq/discourse-telegram-notifications an option to specify the "default thread". If the user sends a direct message to the bot that is not a direct reply to the bot's message on telegram, the bot asks the user if they want to send that message to the designated default thread, or to the thread that relates to the last notifications sent to the chat.
